BLACK HAWK, Colo. — A 37-year-old man who was last seen leaving a casino in Black Hawk has been reported missing, the Golden Police Department said Monday.

Kevin Hazard was seen leaving the Monarch Casino at 11:10 p.m. Thursday, then seen leaving Central City about five minutes later.

Hazard told police he was heading home but he did not return, Golden police said.

He is described as 6-foot-2 and 175 pounds. He was last seen wearing white Nike tennis shoes, a long-sleeve baseball T-shirt that was black with gray sleeves and white/black plaid cargo shorts.

Police said he might also be wearing a white Miami Dolphins hat or an orange New York Mets hat.

Authorities said he was driving a blue 2004 Saturn Ion 3 with damage to the front passenger-side headlight with a Colorado license plate of 883-TRD.

He is believed to be without necessary medication. There are multiple indications he might harm himself, but he is not known to carry weapons.
